ANNUAL WELLNESS VISITS

We offer an all-inclusive AWV program.  We will verify patients eligibility, perform the actual phone or telemedicine visit, and complete the Health Risk Assessment (HRA) and other required screenings.  You will receive all completed documents along with a report of each patient completed and appropriate billing codes for reimbursement.

Reimbursement varies depending on codes used, however we charge a flat rate for this service.
