Blue Ocean Primary Care Solutions 

We offer a complete done for you CCM program.  You will be able to add revenue into your practice with very little work from your providers.  We will determine patients eligibility according to CMS guidelines, contact the patient to obtain consent, then get to work on developing a comprehensive care plan.  Each month, our team will perform and track non face to face time of at least 20 minutes.  
 

You will receive a billing log each month with all patients successfully enrolled and those for which CCM service was completed for the month.  All applicable billing codes will be provided for you to submit for reimbursement.

Reimbursement varies based on codes and time billed.  We charge a flat rate based on codes.
